Two roles that can work together

A rehabilitation medicine doctor, also called a physiatrist or physical medicine and rehabilitation physician, is a medical doctor. AAPM&R describes this specialty as helping people improve function and independence when illness, injury or disability affects daily life. The physician can help coordinate a broader rehabilitation plan.

A physical therapist, also commonly called a physiotherapist, focuses on movement and functional ability. The Philippine Physical Therapy Association's standards describe assessment, individualized goals, interventions and reassessment in collaboration with the patient and rehabilitation team. These are distinct professional roles that may form part of the same person's care.

Start with your referral and your goals

If you were discharged from a hospital or referred after an operation, bring the existing instructions. Tell the receiving clinic whether you have a physician's rehabilitation assessment, a therapy referral, or neither. Ask which appointment should come first for your situation.

PPTA's published standards describe receiving patients through referral from a licensed, registered health professional. Confirm the receiving clinic's current referral and assessment requirements before booking. A rehabilitation physician's consultation and a physical therapy session may be separate appointments with different fees.

Describe a goal in everyday terms: walking to the bathroom, using stairs at home, returning to desk work or carrying a child. The care team can help decide whether it is appropriate and how to measure progress.

Use the right directory category

PDL has separate listings for rehabilitation medicine doctors and physiotherapists. Read the named professional's qualifications and the location's published services. The word 'rehab' in a business name alone does not establish which professional will assess you.

Ask who performs the first assessment, who delivers subsequent sessions and who reviews the overall plan. Confirm that the clinic handles the patient's age group and the type of rehabilitation requested. A clinic offering physical therapy should not automatically be assumed to provide occupational or speech therapy as well.

Check the complete journey into the clinic

  • Ask about the route from the vehicle drop-off to reception, including stairs, ramps, lift access and walking distance.
  • Confirm whether your own wheelchair or walking aid can be accommodated and whether assistance must be arranged in advance.
  • Check toilet access, waiting seating and space for a companion when needed.
  • Ask how long to allow for the first appointment, registration and any separate assessment.
  • For home visits, confirm the service area, named provider, charges and arrangements before sharing an address.

Prepare records and discuss the plan

Bring relevant discharge instructions, the referral, existing imaging reports, a medication list and details of any devices you already use. Ask what clothing or footwear the clinic recommends. Tell the team about restrictions already given by your treating clinician rather than attempting exercises from an online guide.

Before committing to a package, ask what the assessment includes, how goals will be reviewed, which sessions are proposed and what happens if the plan changes. Clarify cancellation terms and whether HMO approval applies to each service. No fixed number of sessions or guaranteed recovery time suits every patient.

Make home instructions understandable

If home activities are prescribed, ask the treating professional to demonstrate them and check your understanding. Confirm what to do if you have difficulty and which symptoms require stopping and seeking advice. Keep the clinic's official contact details and the next review date together.

A successful appointment leaves you knowing who is responsible for each part of care, what the immediate goals are and how progress will be reviewed.
